With our customizable symptom survey, the health of students and staff can be managed more easily, and this survey can be created to fit the school’s needs. We want to make sure your school is safe, and want to do it in the easiest way possible.


Health Screening can be set up by going to Administration, then Health Screening. For new schools, Health Screening can be turned on through the pop up on the administrator’s screen. 

Click Form Questions on the top of the screen to customize the survey questions. A form with up to 5 customizable questions will appear, including 3 default questions. All question prompts can be edited or skipped, including the default questions! The questions on the survey can only be answered with the “Yes” or “No” options. 



Note: Additionally, there is an option to print the form questions for manual input in the case that some parents do not have access to complete the screening in Beehively and you still want to maintain the results for your records.


After clicking Save at the bottom of the question prompt screen, you can select Preview Form to view the layout of the health screening questions. You can even test how the form will respond to different answer selections! Any “Yes” answers will prompt the health screening to give the student a “fail”, and advise them to stay home and seek medical attention. All questions should be designed so that “No” is a pass on the question, and “Yes” is a fail.


At the top of the Set Up page, select Notifications to set up the times to send up to 2 daily reminders to parents through email.


Administrators have to choose the classrooms that will receive the health screening survey emails. If classrooms are not on the list, then the parents and staff will not receive the emails to fill out the survey. You also have the option to send the notifications to specific cohorts. The first email instructs parents to fill out the daily health screening survey, and the second email is a reminder email for users that have not filled out the daily survey yet. The notification times for the initial email and reminder email can be set separately, so that administrators can be certain that parents and staff fill out the survey before every school day. The emails will have a button that links to the survey on the Beehively website. Parents with multiple students will have multiple surveys to fill out, and will receive a reminder email if any of the surveys are skipped.


If notifications are enabled, above the group selection area you will see a status indicator that tells you when the next notification will be sent.


Under the Responses tab at the top of the page, student names and health screening statuses will be available to view. This will help teachers and staff check that students and other staff have filled out and passed the survey. The responses can be filtered through by name, status, and date of submission, as well as organized in ascending or descending order by name, homeroom, user submitted by, survey status, and time received. These responses can also be downloaded as a CSV file.


Users who are not administrators, but have the group-level permission "View Health Screening results for group members" can view health screening results for that group only. For example, this allows an 8th-grade teacher can view results for their 8th-grade class only. (This permission alone does not grant access to Form Questions or Notifications setup).
